many, I was unaware of the preventable medical death toll until my beloved
husband suddenly left this world in 2012. This is one of the last pictures
taken of us.
Yogiraj was recovering from brain surgery after a fall. But, rather than return home, he left this world unexpectedly due to an undetected PE (pulmonary embolism). PE is the #1 preventable medical death in the U.S.A.
Your funding could help a loved one have a better outcome. Professionals, patients and families want to create safe care in hospitals. But research points to issues that challenge their aspirations. In addition to major technological advancements underway, patient safety advocates call for transparency, communication, collaboration, and staff rejuvenation. These are excellent suggestions. At the time of his passing, Yogiraj was writing a book that is a timely contribution to patient safety advocates. This FundDreamer campaign will help me publish this first posthumous book, ‘We’ Can Relate; A Guide for Caregivers. Yogiraj’s death story, 10th Dot’s approach and my unlocked journal using 10th Dot’s tools during the shock and grief, will demonstrate how caregivers can realize their aspirations to give safe care. It will guide professionals, patients, and families - those who receive and give care - toward collaboration, communication, relationship across values, and rejuvenation.

systems they create rarely intend harm. But frequent harm happens when we are
not paying attention to what truly matters. This book and interactive website
is the first in a series demonstrating how to connect the core of care with
SAFE CARE.
