Millions of caregivers suffer needlessly each year.
They carry the emotional burden of an estimated 440,000 patient deaths due to preventable medical errors. Even more people are harmed, often permanently. These professionals navigate a system where communication breakdowns are common, and often play a role in the over 1000 preventable deaths that occur each day.
People and the systems they create rarely intend harm. But frequent harm happens when we are not paying attention to what truly matters. As Yogiraj Charles Bates taught: “Pay attention to what you pay attention to.”
